OFFICIAL PROGRAM INFORMATION:
Describe, in 50 words or less, the company information and products/services to be exhibited exactly as you want the information to appear in the official meeting program book.  The information must be received by the CAC with this application OR before July 30, 2004 to be included in the official program book. If this information is not received in time, your company will be listed in the Program without any description.

By submitting this form, I understand that the Carbohydrate Awareness Council reserves the right to accept or decline this application and that if declined I will receive a refund for any monies paid at the time of reservation.  If application is accepted, I understand this application includes a non-refundable $350 deposit and if exhibition space is cancelled, this deposit will not be refunded.  I have read and agree to the terms and conditions set forth in the Exhibitor Contract and understand that a copy of the contract must be returned via fax or postal mail, signed, on or before August 20, 2004 - failure to return exhibitor contract will result in forfeiture of exhibit space and there will be no refund of monies paid.  By submitting this reservation, I confirm I am authorized and hereby consent for the company to enter into this obligation.


 

 


Below you will find details of exhibit space pricing and contract dates to reserve your booth space.  Exhibition Space includes Expo Floor passes for booth personnel and Conference Registration for one (1) company representative per booth space reserved.

Non-profit organizations and associations* will receive a courtesy rate for a single inline booth of $750. Location is assigned by the Carbohydrate Awareness Council and exhibition space is subject to approval of the Carbohydrate Awareness Council. This space is part of our set-aside program for organizations and associations who wish to participate in our expo & conference.  This space allotment is limited.

Booth pricing includes:  

  • Pipe & Drape (sides and back)
  • Sign with Company Name and Booth Number
  • Reference to your booth on the Expo Floor Map 
  • Company information listed in the Program
  • One (1) Conference Registration per 10x10 - additional registrations available for purchase
  • Ten (10) Expo Hall Admission Tickets for you to give away
  • Upon Request - Table, 2 chairs and wastebasket

Location Selection: Exhibitors are asked to select booth space preferences on their application.  We will try our best to accommodate your request for location, however, final placement is at the discretion of the Carbohydrate Awareness Council, and exhibition space is available subject to the approval of the Carbohydrate Awareness Council.

¹Quad locations provide 3-side walk-around and there is limited availability on the floorplan

IMPORTANT DATES

Early Reservation & Full Payment:  When paid in full, on or before June 30, 2004 - take 10% discount on listed prices above.

All reservations require a $350 non-refundable deposit per 10x10 space. 

All reservations require the exhibitor contract be signed and returned via fax or postal mail.

Open Reservations: Through August 20, 2004

Cancellation Date with Partial Refund: June 30, 2004**

Full Payment Due on or before:  August 20, 2004

*Exhibitor applications from non-members are subject to Carbohydrate Awareness Council approval.

**Cancellations on or before June 30, 2004 will receive a refund of monies paid less the $350 non-refundable deposit. After June 30, 2004 there will be no refunds available for cancellations and no credits issued for future events.
